The lack of information also made it difficult to reconstruct events which occurred prior to and after anomalies.Lesson(s) Learned: To assure efficient and accurate testing, proper communications must occur between test shifts. Written instructions should be kept in a single designa

Adequate information is essential for troubleshooting during testing.Recommendation(s): The information in the shift leaders logbooks needs to be expanded. It should include personnel on shift; a brief description of any events or
