On January 15, 1991 a failure occurred in the 8 foot transonic pressure tunnel during testing of a model wing with attached flap. During the test run, the flap peeled away from the win
3、g, broke free, and proceeded down the tunnel. Extensive damage resulted.Lesson(s) Learned: Torque is not a sufficient measure to assure that joints are assembled with the proper preload.Recommendation(s): While making threaded joints assure that all threads are clean before torquing fasteners.
