2、anel.(480v) While removing cover from above the busses the employee dropped the cover shorting out the busses. An arch and flash occurred slightly burning the employees hands. The main breaker tripped causing an uncontrolled shutdown of the equipment. The interior of the panel was damaged requiring
3、replacement of interior parts.Lesson(s) Learned: Do not allow work to be performed on energized electrical equipment.Recommendation(s): 1. Require Activity Hazard Analysis for all tasks.2. Require Utilities outage request/Permit before working on electrical facilities.3. Do not allow employees to wo
