3、ety and quality audits of SCAPE operations, NASA should ensure that personnel involved are certified so that, when necessary, they can observe the tasks while they are performed.Evidence of Recurrence Control Effectiveness: The Space Shuttle Program concurs that safety and quality audits of SCAPE op

4、erations be performed. However, KSCs position is that NASAs safety and quality personnel monitoring SCAPE tasks will not be exposed to the additional risk of SCAPE operations as personnel can accomplish monitoring tasks by observing and communicating through the audio and video capabilities of the O

5、perational Intercommunication System and operational television (OTV). All SCAPE tasks are conducted on recorded communications channels that are monitored in the control room, and the majority of tasks are observable on the OTV system. NASA quality and safety personnel have performed those audits f
