2、option of a document management and archival policy made it difficult for the document management staff to obtain Investigation Board generated documents for inclusion in the database server.Description of Driving Event: Columbia Accident Investigation Board (CAIB) investigators were initially reluc

3、tant to submit documents to the document management staff (Department of Justice and their contract, Aspen Systems) for inclusion to the CAIB Database Server; they expressed a desire to receive guidelines and direction directly from the Board before providing sensitive documents and work product.Les

4、son(s) Learned: 1. Early in the investigation, the Board did not adopt a document management and archival policy. Consequently, it was difficult for document management staff to obtain CAIB generated documents from the investigators and members of the Board.2. 3. Investigators considered document ma

5、nagement, archiving, and designating documents for release to the public as analogous processes.
