2、led to unnecessary work for activities that were deemed “critical.” The term “critical” in the NPR has a specific meaning. In future programs and projects, correct application of terminology can prevent this. Description of Driving Event: NPR 8715.3, NASA General Safety Program Requirements, defines
3、 “critical” as “A condition that may cause severe injury or occupational illness, or major property damage to facilities, systems, or flight hardware.” However, in ground systems development during the Constellation Program, “critical” was misinterpreted as meaning “important,” which drove a great d
4、eal of unnecessary work relating to risks involving operations, maintenance, testing, logistics, and other disciplines. In addition, it affected the quality of ground and flight safety products at the Project and Subsystem level, because it did not properly communicate risks related to hazardous con
5、ditions. The root cause of this difference stemmed from the interpretation of “critical” to mean “important.” Lesson(s) Learned: It is important to clearly define and correctly apply words, such as “critical”, that can drive work requirements. The usage of the word must be further differentiated bec
6、ause the word bears different meanings depending on how it is used (e.g., operations-critical and mission-critical requirements carry a different weight than do safety-critical requirements). Recommendation(s): In future programs, use the label of “hazardous” (rather than “critical”) as part of a de
