10、nning, administration, allocation, assignment and use of North American Numbering Plan (NANP) numbering resources within the NANP area. All changes to this document shall be made through the INC issue resolution process and adopted by the INC as set forth in the ATIS Operating Procedures. - Notice of Disclaimer Growth history for

12、the previous six months Projected demand for the coming 12 months; and The applicants shall demonstrate that existing numbering resources will exhaust within 6 months. All applicants for growth numbering resources shall achieve a 75% utilization threshold, calculated in accordance with the below cal

13、culation. The numbering resource utilization threshold shall be calculated by dividing all assigned numbers by the total numbering resources in the applicants inventory and multiplying the result by 100. In the case where 100 or more 5XX-NXXs are assigned to a single OCN in a calendar month, the NAN

14、PA shall report to the appropriate regulator(s) the OCN(s) to which the codes were assigned. 14 5.0 RESPONSIBILITIES OF CODE APPLICANTS AND HOLDERS SPs requesting new 5XX-NXX code assignments as well as SPs already assigned PCS 5YY-NXX codes shall comply with the following: 5.1 The code applicant sh

15、all revew a current copy of the Non-Geographic 5XX-NXX Code Assignment Guidelines. 5.2 The code applicant/holder is responsible for meeting all conditions of these guidelines. 5.3 The application process for initial and/or additional 5XX-NXX assignments shall include the following: 5.3.1 Apply to th

16、e Code Administrator and complete all required entries on the Non-Geographic 5XX-NXX Code Forms that are in these guidelines to the best of his/her ability. Applicants who choose to submit their Part A and/or Part C via facsimile or mailed copies will be charged a fee. See the NANPA website at for

17、specific details regarding the associated fee structure. 5.3.2 When requesting growth codes, applicants shall meet the requirements as described in Section 4.10 and conform to the conditions contained therein. On the Part A form, applicants must provide (1) information that provides utilization for

18、the preceding six months and projected monthly utilization for the next twelve months; and (2) the applicants current numbering utilization level. 5.3.3 Provide, if desired, requests for specific and alternative NXX code assignments in order of preference. The Code Administrator will try to satisfy

19、such requests but cannot guarantee specific requests will be satisfied, see Section 6.4.1.1. 5.3.4 When requesting an initial 5XX-NXX code, an applicant shall provide to the Code Administrator with the Part A of the “Non-Geographic 5XX-NXX Code Forms“ documentation demonstrating that the required re

20、gulatory authorization has been obtained for which the code is being requested (See Section 4.3.1). 5.3.5 Agree to abide by sound code conservation practices and the reclamation procedures (see Sections 8 and 7). 5.4 5XX-NXX code holder responsibilities shall include the following: 5.4.1 Assume the

21、responsibility for the assignment of the last four digits of the 5XX-NXX-XXXX numbers and the maintenance of up-to-date assignment records that match 5XX-NXX-XXXX numbers to subscribers/devices for all allocated NXX code(s). These records will be required for audit purposes. 15 5.4.2 Changes in the

22、information associated with a code assignment shall immediately be made known to the Code Administrator using Part A of the Non-Geographic 5XX-NXX Code Forms. Such changes may occur because of the transfer of a code, through merger or acquisition, to a different company. The code holder of the NXX c

23、ode acquired by merger, acquisition or other means must immediately inform the Code Administrator when the change becomes effective. This change information is required to ensure that an accurate record of the entity responsible for the code and the data associated with the code is maintained. If th

24、ere are 50 or more codes that need modifications on intra-company OCN changes, or inter-company OCN changes, a mass modification spreadsheet can be sent to NANPA. Contact NANPA for the most recent spreadsheet. 5.4.3 The resulting SP in a merger/acquisition must revise and provide a new, consolidated

25、 NRUF forecast for numbering resources. The holder of a code assigned by the Administrator or acquired by other means such as transfer (e.g., by merger or acquisition) must use the code consistent with these guidelines. Additionally, the new Code Holder must participate in an audit process, as neces

26、sary. 5.4.4 In order to retain a 5XX-NXX code assignment, it is a requirement of these guidelines and the responsibility of the code holder to cooperate/participate in the audit process. The purpose of an audit is to verify compliance with provisions set forth in these Guidelines. See Section 2.11.

27、5.4.5 A code(s) assigned to an entity, either directly by the Code Administrator or through transfer from another entity, shall be placed in service within six months after the original date of assignment by the Code Administrator for an initial and/or growth code application. See Section 7 and Part

28、 C of Non-Geographic 5XX-NXX Code Forms. 5.4.6 If the Code Administrator determines that a code is not placed in service within six months from the date of assignment, the Code Administrator shall initiate the reclamation procedures (See Section 7). 5.4.7 All code holders shall provide forecasted co

29、de requirements and utilization data to the Code Administrator via semi-annual NRUF reporting (see Section 4.2). Any 5XX-NXXs that are not reported on are subject to reclamation. All NRUF data shall be treated on a proprietary and confidential basis. 5.4.8 Code holders shall immediately return 5XX-N

30、XX code(s) for the reasons stated in Section 7. 5.4.9 Code Holders shall maintain sufficient and auditable data to demonstrate compliance with these guidelines. 6.0 RESPONSIBILITIES OF CODE ADMINISTRATOR The Code Administrator shall: 16 6.1 Provide general information on the structure and limitation

31、s of 5XX-NXX assignments and, on request, copies of the Non-Geographic 5XX-NXX Code Assignment Guidelines, including timely notification of changes. 6.2 Provide code applicants/holders assistance in completing the Non-Geographic 5XX-NXX Code Forms. 6.3 Track NXX code requests and make NXX code assig

32、nments on a first come, first served basis. 6.4 Upon receipt of a Part A, the Code Administrator shall process the application for 5XX-NXX codes and should follow these steps: 6.4.1 Determine if the request is in compliance with code assignment principles and criteria as set forth in these guideline

33、s. 6.4.1.1 Based on the availability of the codes, an attempt will be made to assign the preferred code as listed on the Part A. If two applicants request the same NXX code simultaneously (i.e., the requests are received by the Code Administrator with the same time and date stamp) one of the applica

34、nts would be randomly selected to receive the NXX code. 6.4.1.2 An analysis will be performed to determine if any one code is more appropriate than any other code due to a technical requirement or constraint. 6.4.1.3 The Code Administrator may discuss with the applicant if one of the current codes f

35、rom the existing resource pool is appropriate and select an unassigned code for assignment. 6.4.1.4 Provide a disposition on a NXX code request (See Part B of the Non-Geographic 5XX-NXX Code Forms). There are three possible dispositions; i.e., assigned, denied and requires further information. The C

36、ode Administrator shall respond via the Part B within 14 calendar days from receipt of the assignment request form. The response shall include: a) When assigned: The NXX code(s) actually assigned. b) When denied: The reason(s) for denial and instructions on how and where to make an appeal of the dec

37、ision. c) Requires additional information: The specific additional information required. 6.4.2 If the application is approved and a code(s) is assigned, the Code Administrator shall immediately notify the iconectiv Telecom Routing Administration (TRA) of the assigned NXX code(s) and the code holders

38、 identification information for publication; e.g., in the iconectiv LERGRouting Guide. The TRA shall input the 5XX-NXXs in BIRRDS within five business days of receiving the notification. LERG Routing Guide is a trademark of Telcordia Technologies, Inc. dba iconectiv. 17 6.4.3 Records on codes assign

39、ments and resource availability shall be updated on the NANPA website. 6.4.4 In the case where 100 or more 5XX-NXXs are assigned to a single OCN in a calendar month, the NANPA shall report to the appropriate regulator(s) the OCN(s) to which the codes were assigned. 6.5 Ensure applicant has a current

40、 North American Numbering Plan Numbering Resource Utilization/Forecast (NRUF) Report (See NRUF Reporting Guidelines) on file with the NANPA for the non-geographic 5XX NPA(s). If an NRUF Report is not on file, the Code Administrator shall deny future 5XX code requests from that delinquent SP until a

41、current NRUF Report has been submitted. 6.6 Use historical code assignment data and semi-annual NRUF forecast data to monitor the rate and level of 5XX-NXX code assignments, predict the exhaust of the currently implemented non-geographic 5XX NPAs, and also predict the overall exhaust of all designat

42、ed non-geographic 5XX NPAs. Report these findings, on a semi-annual basis, to the industry (see Section 9.0). 6.7 If one or more NXX codes have not been placed in service within six months from the date of assignment or returned to the Code Administrator, the Code Administrator shall begin the recla

43、mation procedures. 6.8 In the performance of its duties and in meeting its responsibilities, the Code Administrator may encounter situations that may alert them to a possible non-compliance with the industry guidelines which warrants the need for a for cause audit. In these situations, the Code Admi

44、nistrator will inform and forward relevant information which contains the details of the possible infraction to the designated auditor or appropriate regulatory agency for disposition. In addition, the Code Administrator may be required to provide SP-specific data to an auditor in order to facilitat

45、e the audit process. 6.9 Perform reclamation procedures outlined in Section 7, as needed. 7.0 CODE RECLAMATION PROCEDURES Reclamation refers to the process by which SPs are required to return numbering resources to the NANPA. For applications within the United States, the FCC Wireline Competition Bu

46、reau (FCC) has the authority to investigate and determine whether 5XX-NXX code holders have placed into service the 5XX-NXX codes assigned to them within the applicable time frames. Such circumstances are under the direction of the FCC, and the Code Administrator shall abide by the FCCs determinatio

47、n. Service providers shall have an opportunity to explain the circumstances causing the delay in commencing assignment of their numbering resources to the FCC prior to reclamation of the 5XX-NXX code(s). 18 7.1 Resource Assignee Responsibility A 5XX-NXX code holder shall return the resource to the C

48、ode Administrator if the 5XX-NXX code: is no longer needed for the purpose for which it was originally assigned, was assigned for a service that has been discontinued, or was not placed in service within six months from the date of assignment. In the latter case, the assignee may apply to the FCC for an extension date. Such an extension request must include the reason for the delay and a new in service time commitment. 7.2 Code Administrator Responsibility The Code Administrator shall contact any resource assignee(s) ident
