Levels of QualificationThere is one level of qualification for test facilities.Once the test facility has met the requirements of thisstandard, the test facility shall be known as a qualifiedtest facility.5. Test Facility Requirements5.1 Genera
36、l Requirements. The test facility shall havethe following:(1) An organizational structure, including a qualitycontrol program, that enables it to maintain the capabilityto perform satisfactorily the technical functions forwhich qualification is sought.(2) The ability to demonstrate on request from a
37、sses-sors, that it is capable of administering and evaluatingthe required welder qualification tests.(3) A written company policy that staff membersshall have the freedom and authority to identify problemsto a supervisor.(4) An organization in which each staff member isaware of both the extent and l
38、imitation of areas ofresponsibility, and a clear designation of who is the“facility representative.”(5) A facility representative who has the overallresponsibility for the technical operation of the testfacility.(6) Adequate security rules and measures for protec-tion of proprietary and confidential
39、 information.(7) Personnel having responsibility for quality verifi-cation shall be designated by the test facility manage-ment. Personnel shall have access to top management.5.2 Quality Control Program. The test facility shalloperate under a Quality Control Program appropriate tothe type, range, an
40、d volume of work performed. Thisprogram shall be documented in a Quality Control Man-ual to be used by the test facility staff for the purpose ofmaintaining quality control. The manual shall be main-tained current at all times, and shall be available forreview by both internal and external assessors
41、.5.2.1 Manual. The Quality Control Manual shall con-tain information regarding the following:(1) Company Quality Policy.(2) The organizational structure of the personnel ofthe test facility (organizational charts).(3) The operational and functional duties of individu-als performing quality functions
42、 and services pertainingto quality, so that those concerned will know the extentand limits of their responsibilities (job descriptions).(4) General Quality Control Procedures. (These pro-cedures are to include but not be limited to materialtraceability, employee training, employee qualifications,pur
43、chasing, order review, etc.)(5) Quality Control Procedures specific for eachinspection or test including welding procedure specifica-tions (WPSs), as appropriate.(6) Procedures for equipment maintenance andcalibration.AWS B5.4:20053(7) Arrangements for timely feedback and correctiveaction whenever n
44、onconformances are detected.(8) Procedures for assessing customer satisfaction.(9) Provisions for any subcontracted activities(machine shop, nondestructive examination tests) tocomply with the quality control requirements of the testfacility and the methods used by the test facility to verifythe com
45、pliance.(10) Procedures for preparing and reviewing requiredrecords.(11) Assignment of a schedule and an individualresponsible for review of the Quality Control Manual.(12) A record retention policy.(13) Procedure for positive identification of a testcandidate(14) For self-assessed test facilities o
46、nly, the provisionsgoverning assessors.5.2.2 Management Review. The Quality ControlProgram shall be systematically reviewed by, or underthe supervision of, test facility management to assure thecontinued effectiveness of the organization, personnel,and procedures. The review shall be performed at a
47、mini-mum frequency of once a year and shall be documentedwith the results of the corrective actions initiated by thereview and the outcome of previous corrective actions.5.3 Personnel. Test facility personnel shall have the nec-essary education, training, technical knowledge, andexperience for their
48、 functions. The minimum personnelrequirements for the test facility shall be:(1) The inspection and testing services of the testfacility shall be under the direction of a person chargedwith technical managerial responsibility. This personshall be a full-time employee of the facility having atleast f
49、ive documented years of technical experience ininspection and testing of welds.(2) Welder qualification shall be administered by atest supervisor who holds current credentials as a Certi-fied Welding Inspector (CWI) or Senior Certified Weld-ing Inspector (SCWI) under the AWS Standard QC1.Caution: Some jurisdictions may require inspectionpersonnel to be supervised by a professional engineer.(3) This specification requires nondestructive exami-nation (NDE) personnel to be qualified in accordancewith and certified to AWS B5.15, Specification for theQualific
