2、Power Shut Off Capability Description of Driving Event: A painter in the utility annex stepped on a pipe, causing it to fracture at the interface of an attached valve. The pipe remained attached to the valve, but the leak sprayed water onto the motor control center located beneath the pipe, causing

3、a short circuit in the switch gear and a subsequent power outage for several facilities. This power outage resulted in the suspension of test operations that were being performed on the hubble space telescope.Lesson(s) Learned: 1. Utility annex water lines routed above unprotected high voltage elect

4、rical equipment may rupture causing power outages and equipment failures.2. Pipes with less than adequate support will rupture or bend excessively when external forces are applied.3.
