2、 atlas stage thrust section utilizing a 100KV machine. The tubehead was placed inside the thrust area taped into position and exposures were made to the outside. The tape failed to hold the tubehead and it slipped down 6-8 inches hitting a threaded area on a strut. There was only visible damage to t
3、he top of 2-3 threads.Lesson(s) Learned: Heat from hot weather caused the tape used to secure a tubehead assembly to soften allowing the tubehead to fall, resulting in a “close call“.Recommendation(s): Update documentation to require technicians to use rope, web straps or something of that nature in
