2、e a disconnect interface and remove all solid particles (exceeding 25 microns) from the servicing fluids at various system operating pressures, contaminant capacities, and rated flows. Servicing fluids include O2, H2, freon, coolant water, and potable water. Assemblies consist of a quick disconnect,

3、 filter, and associated caps, fittings, seals, and tethered nameplate/identification tags. The QD has internal bellows which permit movement of a probe assembly that opens flight-half coupling. A pod component failure may cause contamination in the orbiter onboard reactants.Lesson(s) Learned: Struct

4、ural failure of pod bellows or other assembly components could cause onboard reactants to be contaminated with gaseous helium and particulates.Recommendation(s): Inspect QD/filter assembly set for cleanliness, loose/missing components, pressure loss/leaks, and damage as part of pre-operational check
