Skylights in the shop area imploded and a considerable amount of debris was drawn into the piping.The principal cause of the failure was deterioration o

Poor welding during assembly and inadequate inspection of the cooler were contributing factors.Lesson(s) Learned: Aging test facility equipment such as deactivated gas coolers that are being used as part of vacuum lines are subject to catastrophic structural failure.Recommen
