、Lessons Learned Entry: 0267Lesson Info:a71 Lesson Number: 0267a71 Lesson Date: 1993-07-07a71 Submitting Organization: JPLa71 Submitted by: B. WagonerSubject: TOPEX/POSEIDON Near-Drop Incident Abstract: In preparing Topex/Poseidon for system test in 1993, the lifting fixture was severely damaged whil

、e lifting the spacecraft. Conduct rigorous analysis to verify and demonstrate that the lifting configuration is stable. Ensure qualified, experienced personnel are on site during critical hazardous operations.Description of Driving Event: As part of the preparation for the testing of the TOPEX/POSEI

、DON spacecraft, the spacecraft and its thermal vacuum fixturing assembly were being lifted and positioned above the thermal vacuum chamber when the assembly began a slow overturning rotation. The assembly rotated approximately 135 degrees before it was halted by the entanglement of the Spacecraft Ho

、rizontal Support Structure (SHSS) with one of the four lifting fixture suspension cables. While resting in this anomalous position, the test team visually determined that the lifting fixture spreader bar (inherited from the Galileo program) had sustained considerable damage and might fail. A decision

、 was made by the Fairchild Systems spacecraft manager to lower the assembly to the chamber floor as quickly as possible to protect the spacecraft and remove the load from the damaged spreader bar. The assembly was lowered to the chamber floor where it was temporarily secured. While no major spacecraf

、rom IHS-,-,-Lesson(s) Learned: 1. The reuse of a residual lifting fixture from previous programs is encouraged, but requires analysis by the current planned user for its application.2. Lifting activities and other critical operations may involve unanticipated risks to spacecraft.Recommendation(s): 1

、. Analysis of the design, load application and stability of a lifting fixture assembly, and the center of gravity of the load to be lifted, must be rigorously performed to verify and demonstrate that the final configuration is stable.2.
