2、, was lifted or hurled, from the top of a flatbed trailer parked on the NASA-Ames Vertical Takeoff and Landing (VTOL) Test Pad. This mishap occurred while the employee was positioning a load suspended in slings on the flatbed. The towbar of the load, which was the flatbed front wheel assembly, rotat
3、ed from a vertical to horizontal position upon being touched or moved by the employee. In the course of being lifted or hurled from the truck by the towbar, the employee sustained serious head injuries and was transported, unconscious, to a hospital. He remained at the hospital for about a month, th
4、en was transferred to Southern California. He was still in a coma when he was transferred.Lesson(s) Learned: 1. Clarify the crew leaders role and responsibility in relationship to the truck drivers role and responsibility.2. Require a more critical inspection by the crew leader of all loads prior to
5、 moving any loads.3. Place more emphasis on center-of-gravity problems/considerations in current crane operating safety courses.4. Since we will probably have to handle this same load again, determine and incorporate a safe means for future handling.5. Continue to furnish, or supplement, advanced tr
6、aining to maintain a high level of safety awareness on the part of all riggers. Consult private industry rigging firms to determine new methods.6.
