2、iver / extractor(HVDE) and a lattice boom mobile crane atop a barge in the Indian River north of Titusville, Florida. Several pilings had already been successfully placed earlier that day. During crane movement to position the final piling, the load on the hook exceeded the cranes capacity and cause

3、d the crane to tip over. A closed-loop process should be used to verify the correctness of data contained in lift plans. Description of Driving Event: On Tuesday, 17 August 2010, a NASA fixed-price contractor was engaged in setting pilings using a hydraulic vibratory driver / extractor(HVDE) and a l

4、attice boom mobile crane atop a barge in the Indian River on the west end of Jay Jay Bridge approximately 2.5 miles north of Titusville, Florida. Several pilings had already been successfully placed earlier that day. During crane movement to position the final piling, the load on the hook exceeded t

5、he cranes capacity and caused the crane to tip over. The load landed in approximately five feet of water along with the cranes boom tip. The lattice boom impacted materials on an adjacent barge and was severely damaged. As it tipped over, the crane slid off its wood matting and came to rest on its s

6、ide with one crawler track and the operators cab up in the air. The operator sustained only minor injuries and climbed out of the cab. No other personnel were injured. Lesson(s) Learned: When using lifting equipment it is important to verify the correctness of data in lift plans as well as the actua

7、l load on site so that incidents like this can be prevented. Recommendation(s): Contractors should institute a process to verify the correctness of data contained in lift plans. NASA should review current requirements relating to extent of oversight / depth of insight to determine if either should b
