This publication has been dev
3、eloped to outline and discuss the elements needed to successfully qualify for listing on the Qualified Suppliers List of Distributors (QSLD). The purpose of the QSLD Program is to establish and maintain a list of pre-qualified sources for certain electronic components that are purchased and managed
4、by the Defense Supply Center, Columbus (DSCC). QSLD products are provided by suppliers that combine accepted commercial practices, quality assurance procedures that are consistent with industry and international quality standards, and tailored when necessary to product-unique requirements that can t

10、-,-4PREFACE The Defense Supply Center Columbus (DSCC) has instituted a Qualified Suppliers List for Distributors (QSLD) Program for products in Federal Stock Class (FSCs) 5961 and 5962. The purpose of this QSLD program is to establish and maintain a list of pre-qualified distributors for certain ful
11、ly competitive products which are purchased and managed by DSCC. The criteria for qualification are tailored along the lines of best commercial business practices. This document contains the criteria and provisions for the program. The Quality Management System (QMS) requirements of the program are
12、contained in Section 3 and 4 of JEDEC standard JESD31, General Requirements for Distributors of Commercial and Military Semiconductor Devices. The JESD31 document may be obtained online at http:/www.jedec.org/Catalog/catalog.cfm. All distributors who wish to participate in this QSLD Program must hav
13、e an assigned Commercial and Government Entity (CAGE) Code and become DSCC qualified. CAGE Codes may be requested online from http:/www.ccr.gov, or call the Central Contractor Registration (CCR) Help Desk at (888) 227-2423. To become qualified, a distributor must satisfy all sections of this documen
14、t and referenced documents. Qualification is valid for 3 years unless terminated or revoked. There is no fee to apply or to become qualified. Only distributors listed on the QSLD will be able to receive awards solicited under this program. Items purchased under the QSLD program will be identified in
15、 the Purchase Order Text (POT). Once qualified and listed as a source on the QSLD, the distributor will be required to adhere to contractual clauses and procurement provisions with the responsible DSCC buying office. 17、ualification for placement on the Qualified Suppliers List for Distributors (QSLD), and the maintenance of QSLD status, requires the distributor to demonstrate that it has in place, and uses on a routine basis, a Quality Management System (QMS) that meets the criteria set forth in JESD31. The object
18、ive of the QSLD Program is to ensure that the distributor routinely controls his processes to provide consistent delivery of products that conform to the contract and specification requirements. Four key elements are required of distributors who wish to be listed on the QSLD. These are: a. Evidence
19、of using a documented Quality Management System which meets DSCCs criteria. b. QPL/QML products having been produced by a Manufacturer who is listed (or was at date of manufacture) on the QPL or QML, and commercial products produced by the original manufacturer, may be purchased under this program I
20、f the products were procured from another distributor, that distributor must be listed on the QSLD. Consequently, all products pursuant to DSCCs contract must be obtained from, or flow through QSLD providers. This closed loop flow must be supported by the providers traceability documentation. No dev
21、iations are permitted under this QSLD program. c. Evidence that product is not commingled; lot identity is maintained. d. The quality of the product is not altered by Distributors. 2.0 SCOPE The products that DSCC procures which are included in this program are certain safety critical and high relia
22、bility items which fall into the Federal Supply Classes of 5961 (semiconductors) and 5962 (monolithic and hybrid microcircuits). See http:/www.dscc.dla.mil/Programs/MilSpec/DocSearch.asp to obtain specifications and standards. 2.1 OBJECTIVE 2.1.1 The objective of the QSLD Program is to establish and
23、 maintain a list of pre-qualified Distributors whose regular use of in-place process controls is designed to ensure delivery of quality products that meet specified requirements, and that participant Distributors likewise control all applicable value-added inventory services associated with defense
24、logistics. The ultimate goals are to improve quality with quality system elements and to reduce product delivery lead times by means of standard quality/process control practices in lieu of certain Government quality assurance provisions and source inspections. 2.1.2 Candidate business distributors
25、are approved for these lists by complying with an established set of quality management and process control requirements, and agreeing to a set of administrative requirements. This Criteria and Provisions document contains these requirements, and is based upon the best commercial business practices
26、for quality control and customer satisfaction. 2.1.3 Distributors ultimately approved for listing under one or more DSCC QSLD program(s) agree, as a condition of continued Qualification, to perpetually maintain their process controls at a level sufficient to meet the QSLD Criteria requirements for a
27、ll qualified commodities. Evidence of non-compliance with any of the QSLD Criteria requirements may be cause for immediate removal. 3.0 CRITERIA 3.1 Quality Management System (QMS) 3.1.1 The Distributors QMS shall continuously meet all the requirements set in JEDEC standard JESD31, or equivalent sys
28、tem as approved by DSCC-VQ. Provided by IHSNot for ResaleNo reproduction or networking permitted without license from IHS-,-,-64.0 PROVISIONS 4.1 Qualification 4.1.1 To obtain and maintain QSLD status, the Distributor must comply with both the technical Criteria of Section 3.0 and the administrative
29、 Provisions of Section 4.0 of this document. 4.1.2 Being listed as a QSLD Distributor does not guarantee award of contracts. Contract award is by open competition amongst qualified Distributors listed on the QSLD. 4.2 General Provisions 4.2.1 The Distributors shall: a. have in-place, maintain and us
30、e a QMS which satisfies all of the criteria and provisions set forth in this document. A copy of the applicants current Quality Manual, reflecting its compliance with the criteria and provisions for QSLD qualification, must be provided to DSCC-VQ with the completed application for qualification. Man
31、ual revisions must be furnished to DSCC-VQ within 15 days of the date of the revision. b. maintain a single QMS; and use a single Quality Control Manual for both its Government business and commercial business. c. possess a Commercial and Government Entity (CAGE) code at the location under quality c
32、ontrol. d. submit its Application for QSLD Requalification to DSCC-VQ at least 120 days prior to expiration of its current qualification. Qualification term shall be three years. e. have in-place a method to identify and verify customer returns. 4.3 Obligations 4.3.1 Government - DSCC will serve as
33、the single Department of Defense (DOD) focal point to consolidate findings and recommend corrective actions for QSLD problems. DSCC will: a. process applications. b. qualify and requalify Distributors c. maintain the Qualified Suppliers List. d. conduct or coordinate site-surveys and audits. e. remo
34、ve Distributors for non-conformance. f. disseminate information to users about non-conforming products. g. make awards only to QSLD listed Distributors or Original Manufacturers of product. h. provide QSLD Distributors with access to a listing of other approved QSLD Distributors. i. reserve the righ
35、t to revert to the basic requirements contained in the original solicitation if the Distributor should be disqualified or removed from the QSLD. 4.3.2 Distributor - The participating Distributor shall assume responsibility to: a. meet all contractual specifications and requirements. There are no exc
36、eptions or waivers unless provided in writing by the contracting officer. b. report any product discrepancies discovered related to DSCC procurements, and corrective actions taken to DSCC-VQ. c. maintain records and documents as indicated in the QSLD Criteria in section 3 and make them available for
37、 examination by DSCC, or DSCCs agent, upon survey or audit. d. permit DSCC, or DSCCs agent, to conduct site surveys and audits as discussed in QSLD provisions 4.5 herein, Surveys and Audits. 38、ate open contract actions with the appropriate DSCC Contracting Officer (C.O.), should you become disqualified from the QSLD prior to delivery. Moreover, you must also coordinate open contract actions with the appropriate DSCC C.O., if you are about to supply to DSCC or DSCCs customer, product that
39、was supplied to you from a Distributor who was removed from the QSLD subsequent to your entering into your contract agreement with DSCC. f. provide other participating Distributors only with material which meets QSLD requirements when notified by the QSLD participant that the material being purchase
40、d is destined to fill a QSLD contract. g. notify DSCC of major changes in Quality Management System, processes, process controls, points of contact or its facility locations prior to implementation. 4.4 Application for Qualification 4.4.1 Application Request - Applications for qualification can be o
41、btained by writing or calling DSCC-VQ, or they can be downloaded from the QSLD Homepage (see preamble). Application packages sent to interested Distributors will include the basic application form and a copy of this document. In order to participate in the QSLD Program, a Distributor must have a CAG
42、E code designation (see Preface for assistance). 4.4.2 Application Processing - The candidate shall submit the completed application to DSCC-VQ (dscc.qslddla.mil) along with a copy of his Quality Manual. The Quality Manual will be evaluated by DSCC for compliance with the QSLD Criteria. The applican
43、t is requested to include references to recent OEM-level or other third party industry surveys or audits of the facility where requested in the application. These references will be evaluated by DSCC and may obviate the need for a separate site-survey. 4.4.3 Application Revision - QSLD companies are
44、 responsible for notifying DSCC-VQ when their product lines or facility locations have changed. Companies shall request and submit a revised signed application once changes have occurred. 4.5 Site-Survey / Audits 4.5.1 When a Distributor applies to be qualified under the QSLD Program, DSCC will cust
45、omarily require a site-survey of the facility. Surveys will include a review of the Distributors QMS and all of the systems and processes that the Distributor is required to have in-place and in-use, under this document and JESD31. The purpose of a facility audit is to ensure that the Distributor ha
46、s in-place and in daily use, a Quality Management System which conforms to the requirements of the Criteria and Provisions of the QSLD Program, as reflected in this document. An audit will involve the examination of applicable documents (including traceability records), processes and procedures, as
47、well as the various systems required for attainment of qualification. 4.5.2 Industry or other third party surveys or audits may be considered by DSCC in the review of the Distributors Application for Qualification. Such surveys or audits may be used by DSCC in lieu of, or in addition to, QSLD site-s
48、urvey requirements. 4.5.3 After qualification in the program, DSCC or DSCCs agent may conduct random announced or unannounced audits of a Distributors facility to confirm adherence to the QSLD Criteria. Audits may be an on-going policy during the life in the QSLD Program. All audits are performed at
49、 no charge to the Distributor. During audits, random sample collection of the Distributors product shall be allowed for the purpose of independent laboratory testing. Thus, DSCC may pull samples for later testing against specification or contract requirement. The Government will pay for the cost of such tests and the Distributors expense is limited only to the cost of a small quantity of samples
