2、vely and economically, an investigation board may use outside contractors to ensure a perception of “independence.“Description of Driving Event: There was concern at NASA over the role of the Columbia Accident Investigation Boards procurement of goods and services through a support contractor. NASA
3、and other government agencies have the ability on occasion to provide services much cheaper. Procurement decisions were often made on the basis of the perception of independence rather than cost efficiency or effectiveness.Lesson(s) Learned: In general, an independent accident investigation board ma
