7、to defined quality criteria.3.3 Testing Agency Quality Systemthe organizationalstructure, staff responsibilities, policies, standard operatingprocedures, and processes which, when implemented, assist thetesting agency in achieving its quality objectives.3.4 Evaluation Authorityan independent entity,

8、 apart fromthe testing laboratory being evaluated, that has the capability toprovide an unbiased evaluation of the professional and testingactivities of testing laboratories.3.5 Agencythe organization engaged to test or inspect, orboth, road and paving materials as required by a specificationor cont

9、ract.4. Significance and Use4.1 This document supplements ANSI/ISO/IEC 17011 andtogether these two documents provide the minimum require-ments for an accreditation body which accredits agenciestesting and inspecting road and paving materials.4.2 The user is cautioned that accreditation granted by an

10、accreditation body can only serve a useful purpose if theprocedures followed to obtain the accreditation are practicedon a daily basis by the testing agency. It is not the existence ofthe system but the routine implementation of the proceduresand policies of the quality system that is important.5. A

11、ccreditation Body5.1 The accreditation body in developing and implementingits accreditation program shall meet the requirements inANSI/ISO/IEC 17011 and the requirements of this standard.6. Personnel6.1 The accreditation body shall document the duties, re-sponsibilities and authorities of top manage

12、ment who couldaffect the quality of the accreditation process.6.2 The accreditation body shall ensure that personnel havethe necessary and proper education, experience, training, andtechnical knowledge.6.3 The accreditation body shall describe the requiredqualifications, education, and experience fo

United States16.4 The accreditation body shall ensure that the assessorsthey employ are familiar with the accreditation process andaccreditation criteria.6.5 The assessors shall be trained through a formal assessortraining program that ve

16、rifies an assessors technical qualifi-cations. The assessors shall be trained in the use of the methodsand documents used for assessments. Training records shall bemaintained by the accreditation body.6.6 The assessors shall be thoroughly trained in the qualitymanagement system requirements and have

17、 a thorough knowl-edge of the test methods which they are assessing. Trainingrecords shall be maintained by the accrediting body.6.7 The assessors shall not perform any assessment if aconflict of interest exists between the assessor and the labora-tory.6.8 The accreditation body shall evaluate each

18、assessorsperformance. The evaluations shall consist of an on-siteobservation or peer monitoring in conjunction with the reviewof either the assessors feedback from laboratories or a reviewof assessment reports. These reviews shall be conducted atleast once every three 3 years. Evaluation records sha

19、ll bemaintained by the accreditation body.6.9 The accreditation body shall, as a minimum, holdmeetings at least annually with assessors to discuss changes inprocedures, provide training to ensure consistency inassessments, present any issues noted during the annual man-agement review that apply to t

20、he assessments, and other topicsthat would assist the assessors in improving the quality of theirservices.7. Accreditation Process7.1 The accreditation body shall conduct on-site assess-ments that include a detailed review of test procedures, testingequipment, and quality systems. This assessment sh

21、all verifythat the testing agency has the appropriate equipment, calibra-tion and verification records, test procedures, and trainedpersonnel to perform every procedure for which the laboratoryhas requested accreditation.7.2 The accreditation body shall conduct on-site assess-ments at 24-month inter

22、vals. If the accreditation body performsannual surveillance or desk audits, the interval for on-siteassessments may be increased to a maximum of 30 months.NOTE 1The accreditation body may use designated evaluation au-thorities to perform some or all of the on-site assessments required foraccreditati

23、on provided the accreditation body has documented that theevaluation authority meets all the requirements of this standard.7.3 The on-site assessment shall include a demonstration ofeach test method or practice for which the testing agency hasrequested accreditation. At the discretion of the assesso

24、r, it ispermitted for some procedural aspects of the test methods orpractices to be described by laboratory personnel in lieu ofdemonstration. The test methods shall be performed by tech-nicians employed by the laboratory at the specific locationseeking accreditation.7.4 The assessor shall verify th

25、at the equipment that thelaboratory uses to perform the testing that is listed in theproposed scope of accreditation complies with the specifica-tions listed within the test standards. This accreditation bodyshall verify the compliance of the laboratorys test equipmentwith its own equipment that, wh

26、ere practical, has been cali-brated by an agency which complies with ISO/IEC 17025(Note 2). The assessor has the option of either physicallyverifying the test equipment or observing laboratory personnelphysically verifying the test equipment with the equipment thatis owned or controlled by the accre

27、ditation body. A review ofthe laboratorys equipment records would satisfy this require-ment if the equipment has been calibrated or verified by acalibration agency which satisfies the requirements of ISO/IEC17025.NOTE 2Certain equipment used in verifications is not readily cali-brated such as tape m

28、easures, feeler gauges, and straight edges. Equip-ment such as temperature measuring devices, calipers, pressure gauges,can be readily calibrated.7.5 The on-site assessment shall include a review of thetesting agencys quality system. The testing agencys qualitysystem as a minimum shall conform to th

29、e requirements ofASTM D36667.6 The accreditation body shall require that the testingagency return an initial response within 30 days from the dateof the assessment report. This response shall include a recordof investigations made and corrective action taken.All findingsnoted in the assessment repor

30、ts issued to the testing agencyshall be completely resolved within 90 days from the assess-ment report date.8. Proficiency Testing Requirements8.1 The accreditation body shall verify and document thatthe testing agency participates in applicable proficiency sampleprograms for those tests that the la

31、boratory is requestingaccreditation, when such programs exist. The proficiencysample program shall meet the following minimum criteria:8.1.1 Includes a minimum of 30 participants, each repre-senting a different laboratory and different technicians.8.1.2 Issues rating(s) for the test method(s) covere

32、d.8.1.3 Covers one or more of the test methods for which thetesting agency requested accreditation.8.1.4 Is independent of the participating laboratories.8.1.5 Distributes samples at least once annually.8.2 The accreditation body shall require that the testingagency complete a record of investigatio

33、ns made and correc-tive action taken to address any proficiency sample result thatwas greater than 2 standard deviations from the averageobtained by the participating laboratories.9. Assessment Reports9.1 The accreditation body or their designated evaluationauthority shall issue a detailed assessmen

34、t report on adherenceto standards by the testing agency as evidenced by the testingagencys quality manual and verified by the assessor for allstandards under which the testing agency has requested ac-creditation. The assessment report shall include as a minimumthe following:9.1.1 Standards / Test Me

35、thods / Practices which werecovered during the on-site assessment.D7495 1229.1.2 Nonconformities/findings which were observed dur-ing the on-site assessment.9.1.3 Date of Assessment.9.1.4 Name of Assessor.10. Granting Accreditation10.1 The accreditation body shall, prior to making adecision, be sati

36、sfied that all the requirements have beensatisfied.10.2 If all requirements have been met, the accreditationbody shall issue an accreditation certificate or shall otherwisepublicize accreditation details including:10.2.1 Identity and logo of accreditation body.10.2.2 Identity and location of the tes

37、ting agency.10.2.3 Unique identification number of the testing agency.10.2.4 Effective date of granting accreditation.10.2.5 Accreditation expiration date.10.2.6 Scope of accreditation including each test method,practice or other standard.10.2.7 Reference to the standard upon which the accredita-tio

38、n was based.10.3 The accreditation body shall maintain a directory thatincludes all accredited laboratories. The directory shall includethe following:10.3.1 Name and location of each laboratory.10.3.2 Dates of granting accreditation.10.3.3 Accreditation expiration date.10.3.4 Scopes of accreditation

39、 including each test method,practice or other standard.11.
