6、 of theexamination; identifying information of the examiner; theresults of the examination; the instrumentation utilized tocollect the test data; and any explanatory remarks determinednecessary by the examiner. For a specific issue examination, allrelevant questions and the examinees answers shall b
7、e in-cluded.3.1.4 quality controla systematic, independent, and objec-tive review, evaluation, and critique of a PDD examination bya qualified examiner. This review is designed to ensure eachexaminer maintains professional standards.4. Summary of Practice4.1 All PDD examinations may be subjected to
8、a qualitycontrol review. Prior to a PDD examination being consideredby any court or judicial tribunal, in order to ensure eachexamination meets appropriate standards, each examinationshould undergo a quality control review.4.2 Experienced examiners who are trained in the PDDformat utilized by the or
9、iginal examiner are qualified toconduct a quality control review of an examination.4.3 A quality control review shall be independent andobjective and meet all applicable ASTM standards. The qualitycontrol process should involve, but is not limited to a reviewof: the test data analysis of all charts
10、collected during theexamination; all questions and answers utilized during theexamination to produce the data which were evaluated; thePDD report; any available electronic recordings; and all alliedpaperwork completed by the PDD examiner in support of theexamination.4.4 The person conducting the qua
11、lity control review shallattest, in writing, as to whether the test data supports theconclusion rendered by the original examiner.4.5 All PDD documents and test data shall remain availablefor a quality control review for no less than a year or until theincident or circumstance requiring the examinat
12、ion to beconducted has been completely adjudicated.5.
