21、k Application forms are also available on request from the registration bodies. In the absence of a registration authority at national level, card issuers in that country shail contact their national standard body The national standards body shall either set up a remon authority at national level or
22、 make an agreement with another national stan- body to act as the regishation authority at national level for their nationai card issum. 6.2 Criteria for approval and rejection of applications 6.2.1 Criteria for approval of an application for a sinale ILN Applications for a singie IIN shali meet ail
23、 the criteria for approval below and shall not comply with any of the criteria for rejection in 6.2.2. The criteria for approval are: I I a) the applicant applying for a singe IIN with M = 80 shall not alrea4y have anIIN with MII = 80 assigned to it in its own right; b) the IIN with MIi= 80 shali be
24、 for immediate use, within 12 months of the date of issue; c) the applicant shall be a single entity with legal -; d) the applicant shall use the IIN for canis used for heaith care, heaith care coverage or heaith care entitlement; e) the applicant shall pay the corresponding fee required by the rewo
25、n authority. NOTE. A group of mutually-independent issuers in several member countries may agree to form an aiiiance under a single entity to issue uniform car b) the applicant has previously been assigned an DN with Mil = 80 in its own right; c) the IIN will be used to identify or differentiate bet
26、ween products, services or proprietary rights, or solely to facilitate routing, or to identify geographic location; d) the IIN will be used to identify or differentiate between branches or subsidiaries within the applicants organhiion; e) the applicant has made a request which is outside the scope o
27、f this European Standard; f) no card wiil be issued for immediate use, e.g. within 12 months hm the b) to notify the applicant and the registration authority of their decision; c) in case of approval of the application, the appeal body should instruct the regishation authority to issue an EN to the
28、applicant; d) to respond to general enquiries covering this European Standard Appeal agajnst rejection of an application shall be lodged with the appropriate body wih 90 days of the date of the leer of rejection. O BSI 1997 6.2.3.3 Informution to be provied Where an application for a single IIN has
29、been rejected by the regidration authority at national level, the following information shall be provided by the applicant to the relevant appeal body in support of the appeal: a) statement of which rejection cause (see 6.2.2) is disputed and why the applicant believes that the rejected application
30、fullils the criteria for approval (see 6.2.1); b) statement of special circumstances whereby a specific requirement of the rejected application can be met, but such requirement is outside the current proceures and criteria for approval outne in this European Standard 6.2.4 Responsibilities #card iss
31、uing applicants The responsibilities of card issuing applicants shail be: a) to comply fully with the numbering system and the procedures for application for IINs as contained inthisstandard; b) to retain the completed application form containing the IiN assigned to the applicant by the registration
32、 authority at nationai levei; c) to issue cards using the IIN assigned by the regidration authority within 12 months of the date of assignment of the IIN, d) to communicak to the registration authority any proposed changes which would alter materiai facts contained within the original application. 6
33、.2.6 Registration authorit at nutiona level 6.2.6.1 hligibility to become a registmtion authoritg at national hei The registration authority at national level shall be a single entity designated in each country The following bodies may act as registration authority at national level in processing ap
34、plications for IINs: a) the national member body of CEN or an agent appointed by them to act on their behalf; b) the national standards body of another member of CEN, upon a bilateral agreement, if the national standards body is unable to establish a national numbering system. A document entitled Gu
35、idance for registration authorities at national level (annex C) is available to assist registration authorities at national level in assessing the eligibility of applications and to assist in the procedures for dealing with applications. This document is available from the secretariat of the registr
36、ation authority at European level. Page 6 EN 1867 : 1997 6.2.6.2 Resignation if a registration authority at national level which is an agent appointed by the national standards body finds it necessary to resign, six months notice shall be given to the national standards body. If the regidration auth
37、ority is the member body of CEN it shall make bilateral agreement with another member body of CEN. AU changes shall be notified to the registration authority at European level. 6.2.6.3 ResponsiMities The responsibilities of a registration authority at national level shall be: a) to ensure that the a
38、pplication fuJly complies with the numbering system and procedures for application for IWs in this standard; b) to verify that the applicants card use, as described on the application form, complies with the scope of EN 1387 : 1996, c) to process, within 60 days of receipt of the application, applic
39、ations for IINs from within their countries or areas of responsibility; d) to send notification to the applicant in writmg, within the same period of 60 days of receipt of the applicalion, as to the disposition of their appiiaion; e) to assign a unique IiN to each card issuer; f) to retain a copy of
40、 each application form; g) to respond to enquiries from the appeal authoriw, NOTE. if it appears to the registration authority at national level that the application contravenes national niles, the registration authority at national level should notify the appeal body accordingly. h) to inform appli
41、cants whose applications have been rejected of their right to appeal and the details of the appeal body; i) to send a copy of each rejected application to the appeal body for information in case of a later apProV4 j) to provide a yearly report of activity to the registration authority at European le
42、vel. The yearly report shall include information on the number of IINs assigned and the number of requests for the national register. 43、0bbY277 013 Page 6 EN 1867 : 1997 6.2.6.4 National register of card Issuer Ichmtification Numbers 6.2.6.4.1 Publicacion and avaWity The registraton authority at national level shall maintain a database of information taken directly from the application form. Au infomation in the database (whether pu
44、blished or not) is considered confidential. Based on the infodon contained in this database, the registraton authority shall publish a register of card issuer identification numbers. The register shai be published in both numerid and alphabetical order. The register of card issuer identification num
45、bers shali be a publicly available document. It is available from, and may be charged for by the registration authority at national level. 6.2.6.4.2 Contents The register of d issuer identication numbers shall contain the foilowing irormatiom a) name of card issuer, b) address and telephone of card
46、issuer and principal contacts within organhation as indim on the application form; c) IIN assigned to the card issuer by the registration authority at national level; d) date of end of issuing, if any NOTE 1. A copy of each application received will be maintained on file by the regishation authority
47、 at national level. NOTE 2. The card issuer is responsible for advising the regislration authority at national level of any changes to information on the application (whether part of the register of card issuer idenMication numbers or not). 7 Registration authority at European level 7.1 General The
48、registmtion authority at European level is the secretariat of the CEN Technical Committee responsible for EN 1867. 7.2 Responsibilities 7.2.1 General The responsibilities of the registraton authority at European level shall be: a) to maintain the database of registmiion authorities at nationai level
49、; b) to circulate a copy of the database of registration authorities at nationai level to ali registraton authorities at national level and to the ISO/IEC 7812 regishation authority. 7.2.2 Coordination The registmtion authority at European level shali have the opportunity, at the request of the registraton authorities at national level, to coordinate the practice of issuing numbers at national level, foilowing examinati on of registratons submitted to it for that purpose.
