3、the Columbia Accident Investigation Board (CAIB) were not numbered with unique page numbers (endorsed numbers). Consequently, document management support staff had difficulty in tracing hard copy documents back to their electronic versions in PBMA.Lesson(s) Learned: Since the electronic and hard cop
4、y documents submitted by CTF to the CAIB were not numbered, document management staff were required to electronically endorse all electronic files, and hand number label all hard copy files for PBMA and CAIB generated documents. This resulted in duplicate documents being unnecessarily entered into t
