2、ing Event: During an engineering test, organizations external to the test organization were performing maintenance activities in the test vicinity. Welding was being performed in a test facility, which resulted in the deactivation of the facilitys fire alarm bells during the first few days of a test
3、. This condition was discovered by chance observation and not during inspection nor during normal pre-test activities.Lesson(s) Learned: 1. Coordination of discrete operations in a facility is needed to preclude the deactivation of required emergency systems especially during a hazardous operation o
4、r test activity.2. Coordination must be an integrated process involving at least one person knowledgeable in the facilitys planned day to day operations.Recommendation(s): 1.
